Decisions, Decisions, Decisions.....

We have been working on a Medical Questionnaire and a Openness Questionnaire. I thought I'd share some of the questions from these. I know that I've said it before but there are a lot of things to think about during the adoption process. Some of these I had thought of, some of them I hadn't. We have to answer Yes, No or Will Consider.
Openness Questionnaire
Birthparent Contact
1. Third Party assisted meeting with the birthparents and/or possibly with extended family before child's birth
2. Exchange letters before birth
3. Birthparent participation in naming of the child
4. Meeting with birthparents after termination of parental rights
5. Presentation of child by birthparents at the placement ceremony
6. Share gifts with birthparents at placement
7. Receive gift for the child from the birthparents
Child Placement
1. Direct placement in your home from the hospital, according to applicable county/state law
2. Hospital contact with birthparent
3. Legal Risk placement
4. Presence at delivery or during visiting hours
5. Interstate placement/willing to travel
Birthparents Involvement After Adoption (as allowed by state law)
1. Pictures and written updates:
a. Beyond First year following placement
b. For 18 years (most commonly done 1 or 2 times per year)
2. Visitation between birthparents, child and adoptive parents:
a. Only during the first year after placement
b. For a specified period of time
c. Throughout child's life
Do you feel reasonably comfortable with Bethany's policy regarding:

Medical Questionnaire
1. Age of Child: 0-3 months, 3-6 months, 6-9 months 9+ months, toddler

4. Surgically correctable medical condition such as hernia, turned in feet, heart defect, cleft palate
5. Permanent medical condition such as CP, Cystic Fibrosis, heart defect, seizure disorder
6. Vision, hearing, orthopedic impairment
Medical History of Birthparent
1. Limited information on birthfather
2. No information on birthfather
3. Victim of Date Rape
4. Victim of Incest
5. Mental Illness in birthparent
6. Mental illness in extended family
7. Mental impairment in birthparent
8. Mental impairment in extended family
9. Learning disablility in birthparent
10. Learning disability in extended family
11. Drug/alcohol us in birthmother before pregnancy
12. Drug/alcohol use in firthfather history
13. Drug use in extended family
14. Antisocial behavior/criminality
15. Viral Sexually transmitted diseases
16. HIV infection in birthmother
Birthmothers Prenatal Care
1. No prenatal care
2. Lacked consistent medical care
3. Early minor alcohol usage
